    • The Silent Witness Report Form is designed to allow individuals to report incidents they are aware of and remain anonymous if they so choose. If you have information you believe university officials should know about, fill out the Silent Witness Report Form and university officials will follow up.
  • Submit an EOTIX Report | Minnesota State University, Mankato
    • The Office of Equal Opportunity & Title IX is responsible for providing services to faculty, staff, and students who believe they have been harassed or discriminated against because they are a member of a protected class.
