Overview
Employees can conveniently access their W-2 and 1095-C tax forms electronically through the Self Service portal as soon as they become available in January, eliminating the wait for mailed copies. This secure option allows users to download, print, and save current and past forms anytime. All current and former employees are eligible, even if they previously declined electronic consent. To receive forms electronically, employees must provide separate consent for each form in Self Service. Additionally, it’s important to keep address information accurate in both Self Service and Workday to ensure proper recordkeeping.

Am I eligible?
Yes. All current and former employees can access their W-2 and 1095-C forms electronically in Self Service, even if they originally declined to give consent to receive them electronically.
How do I access my W-2 and 1095-C forms electronically?
You must consent separately to receive each form electronically. Follow the steps below to give consent to receive your W-2 and/or 1095-C electronically in Self Service.
NOTE: If you’ve already consented to receive these forms electronically, you don’t need to do anything.
Give consent to receive your W-2 form electronically:
- Sign into Self Service Portal (https://mn.gov/selfservice), using the same user ID and password that you use to access your paystub.
- Select Self Service tile.
- On the Self Service Landing page, select Tax Forms tile.
- On the Tax Forms page, select View W-2 Forms tile.
- A pop-up box will appear allowing you to consent to receive the W-2 form electronically. Once you give consent, you give consent, you can access W-2s for previous years immediately and for 2022 as soon as it is available. If consent is given before W-2s are printed by the state, a W-2 will not be mailed. The pop-up box will not appear if you have already consented.
Give consent to receive your 1095-C form electronically:
- Sign into Self Service Portal (https://mn.gov/selfservice), using the same user ID and password that you use to access your paystub.
- Select the Self Service tile.
- Select Benefits tile.
- Select Insurance tile.
- Select Form 1095-C Consent option in the Navigation Collection on the left.
- A pop-up box will appear allowing you to consent to receive the 1095-C form electronically. Once you give consent, you can access 1095-Cs for previous years immediately and for 2022 as soon as it is available. If consent is given before 1095-Cs are printed by the state, a 1095-C will not be mailed.
Need to update your Address?
Please follow the instructions below on how to update your current address to ensure your information in our systems is accurate.
Self Service
Change or view your address in Self Service by selecting the About Me tile > My Contact Information tile > Addresses.
Please ensure to have the correct information for each required field:
- Country
- Address Line 1
- Address Line 2 – if you have an apartment number, suite number, building names, or PO box numbers
- City
- State
- Postal Code
- County
Workday
Please ensure that your address is correct in Workday. After you log in to Workday, click on the Profile (person) button in the upper right corner, then click View Profile. Next, click on your profile’s Contact section.
Review your Home Contact Information, and if your contact information is accurate, no further action is needed. If your Primary Address is incorrect, update it in Workday.
Instructions for updating Address information:
- Click the Edit button.
- Use either Edit (pencil) button to update your profile with the correct address.
- Please ensure to have the correct information for each required field:
- Country
- Address Line 1
- Address Line 2 – if you have an apartment number, suite number, building names, or PO box numbers
- City
- State
- Postal Code
- County
- Be sure to click the Submit at the bottom of the page to save your changes.
