Overview
To update your name, date of birth, or Social Security/ITIN number at Minnesota State University, Mankato, you must complete the Personal Data Change (PDC) form and submit appropriate documentation. Acceptable documents include a driver’s license, passport, marriage or divorce decree, court order, or certificate of citizenship. If you're receiving financial aid or updating your SSN/ITIN, a signed copy of both sides of your Social Security or ITIN card is required. You can submit the form and documents in person, via secure upload, email, or regular mail, and updates will reflect in systems like D2L and MavCard within a few days.

Name Change
To change your name or make corrections to your date of birth or social security number, please complete and return the Personal Data Change (PDC) form.
Please provide a clean/legible copy of one of the following documents:
- Current Driver’s License (showing your new last name)
- Current Passport (showing your new last name)
- Marriage Decree
- Divorce Decree
- Court Order
- Certificate of U.S. Citizenship
- If you are receiving financial aid, we also need a copy of both sides of your signed social security card
Date of Birth
Use the PDC form to correct or add your date of birth. The documentation needed is a current passport, driver’s license, or birth certificate.
Social Security Number or ITIN Number
Use the PDC form to correct or add your social security/ITIN number as well. Please provide a copy of both sides of your SIGNED social security or ITIN card.
Options to Submit PDC Form and Documentation:
- Bring your documentation along with your completed PDC Form to the Campus Hub or the Registration and Academic Records window. Either place can make copies of your documents.
- Use MoveItSecurely which is accessible to anyone with a Star ID and password
- Send your copies via regular US mail to: Registration and Academic Records; Minnesota State University, Mankato; 132 Wigley Administration Center, Mankato, MN 56001.
- Email your forms to Registration and Academic Records and select Name Changes from the Topic menu and use the Attach Documents to upload your forms.
D2L and MavCard Info
Once your name is changed, it will filter to the D2L and MavCard systems within a few days. If you want a new MavCard, please contact the MavCard office directly at mavcardoffice@mnsu.edu.
Email Info
Registration and Academic Records has no access to change your campus email address. If you want your email address changed to match your legal name, please submit a ticket to IT Solutions and request that they update your email address. Inform ITS your name has been changed in the student records system.
Name on Diploma
If you are graduating soon, please contact the graduation staff at graduation@mnsu.edu to make sure your name is printed properly on your diploma.
